Thank you for purchasing a Tree of Life leaf plaque to honor a special event or occasion. These plaques are not intended for memorial gifts. Each leaf is a $180 donation and may take up to 8 weeks to complete. Please complete a new form for each leaf plaque you wish to order. 

New leaf plaques will be installed after the completion of the Building Dreams project in early 2025.  If you have any questions, please contact Audrey at audrey@cbbsb.org.
