There is currently a bug impacting this form! We are waiting for a fix. In the meantime, please choose your answers to multiple choice questions carefully. To fill out the form you will need:
-medical insurance information for each child
-doctor name and phone number
-emergency contacts, at least one local and one out of state, with a cell phone for each

Please note that you cannot save your progress on this form and return to it later! Please make sure you set aside enough time to fill out the form to completion (about 10-15 minutes per child), and ensure you have the information listed above easily available.

You must submit a payment for this form to be submitted. If you wish to pay by check, please set up a "payment plan" and pay the first installation to submit the form. You can then pay the remaining balance by sending a check in to our bookkeeper, Terry Grimes, and she will remove the payment plan.

 
Sibling Discount and Early Bird Discount: No code is needed to receive either of these discounts.  You will automatically receive the Sibling Discount of 5% off total tuition if you are enrolling two or more children in JLP, and you will automatically receive the Early Bird Discount of 5% off total tuition if you submit this registration by August 1.  You can receive both of these discounts, if you register two or more students by Aug. 1!
 
CBB Temple Membership is required for all families enrolled in our Jewish Learning Programs. Membership is valid when membership commitment is made. If you need to renew your membership, or if you are not currently a CBB member and you would like to enroll your child in our Jewish Learning Programs, you must submit your membership renewal or new member form within 30 days of filling out this form.

It is CBB’s policy to admit all members’ children to its Jewish Learning Programs regardless of ability to pay. If you need financial assistance, please submit a separate Financial Aid Request form. Membership

Our Board of Trustees sets a recommended yearly membership gift. Many give at this level, some give an alternative amount. Those who are able, give above the recommended levels. This generosity sustains our temple in many important ways, including allowing young families, and members on fixed or lower incomes to fully participate in congregational life. No one is ever turned away for financial reasons.

Below you will see the recommended yearly membership gift levels, including the "alternative" option which allows you to select your own donation level.

Our membership year is July 1 - June 30.

Membership contributions provide nearly half of CBB’s annual operating budget. Leadership Donors ensure that we keep our doors open for all to participate regardless of financial situation. Leadership Donors are invited to special gatherings and receive recognition throughout the year.

We may offer the Mechina College Class track again next year, in the '26-'27 school year.  If you believe your child would be interested in taking the Mechina College Class in the '26-'27 school year, please check the box below.

Sibling Discount

The Sibling Discount will be automatically applied!  Note that you will not see the discounted tuition come off the total, as the total below already reflects the discounted rate.  If you have any questions about the discounts, please email Leah, Asst. Dir. of JLP, at leah@cbbsb.org after submitting your registration form.


Financial Aid

If you are receiving financial aid, you will input your financial aid code on the following page.  Please paste your financial aid code in the Discount Code box on the following page.  The total will be adjusted when you hit "Confirm and Continue."
 


If you would like to set up a payment plan to spread out this total over several months, you will be able to do so on the next page. There is no additional fee when using a payment plan.
